Implementation of A Digital Tool for Survivorship Care

Much progress has been made in the prevention and treatment of childhood cancers. However, a growing population of pediatric cancer survivors have called for a rethink on how to improve and optimize the quality of survivorship.
The Psychosocial & Survivorship research theme aims to develop an interactive evidence-informed platform for cancer patients and survivors. This platform will include a repository of resources for symptom reporting and management, resources on survivorship education and tools for on-going survivorship care.
This platform will be designed in close collaboration with patients and survivors of cancer to help guide the user experience and provide content to adequately address the needs of the community. The care plan tool will be based on Children’s Oncology Group (COG) Long-Term Follow-Up Guidelines.
Moreover, the project will engage healthcare providers and survivors to co-develop a patient-centred national framework to address barriers in survivorship care, build on current transition and survivorship care models, and assess gaps in care regardless of whether they receive care in a cancer centre or in the community.
The impact is of this project is substantial and addresses a crucial gap across Canada – through this project the theme will be able to enhance coordination of survivorship care for all.
